How exactly do I write a letter requesting information?

I want to request information from the FOIA as regards to Flight 19. Mainly the transmission. (I've seen parts of it) but I want to the whole thing. Maybe all I can get on the topic other than what I've read in books, or seen on T.V.

Also who the letter is supposed to be addressed too.
Why are you hiding the truth from me when I can see it in plain sight? The only thing this proves is how blind you really are.
